Even if by some miraculous measure we can completely cut off all the possible ways that HIV AIDS can be spread, we will still see a sharp increase in the number of people with HIV AIDS five years from now.
This is the statement made by Doctor Suharto, SpKO, DPH from National AIDS Commission. He also said that only Thailand and Australia have successfully managed to control the increase rate of the number of people with HIV AIDS. Indonesia, like many other countries in Asia, is still struggling with the horrific number of new HIV AIDS cases every year.

Based on a report from the Ministry of Health, until September 3338 people have been infected by HIV, with 2363 confirmed AIDS cases. It is also reported that the biggest cause of HIV AIDS transmission is not heterosexual intercourse (39.7%), or homosexual intercourse (6.2%), but the use of syringe to inject drugs (40.5%). Use Drug at Low Dosage

Now here’s the thing. The US courts have been at work, accepting cases to prove every possible type of danger caused to humankind by every possible product. Whether it’s cars that just will not stop no matter how many times you stand on the brakes, or the attack of killer tomatoes full of salmonella, the cases just keep on coming. It’s not surprising the political parties wrestle with the idea of cutting back on the right of trial lawyers to harass successful businesses. In fact, every week you see stories of manufacturers surrendering in the face of legal challenges. Sometimes, right or wrong, it’s cheaper just to give up and walk away. So what’s the story here?

We have to cross to seas to distant islands going under the name of New Zealand. Even though these people live too far away to be considered properly civilized – no developed court system for suing manufacturers marks them out as still primitive – they have witch doctors who have been studying the effect of the key drug used to treat acne. Remarkable though this may seem, they actually managed to round up 1,700 people who have been taking this drug and followed their health for six years. All participants had severe acne that had failed to respond to the usual treatments – rubbing the skin with live toads was the standard. They all took this fierce Western drug in low doses and, remarkably, there did not seem to be any serious side effects. A few had slight cracking of the skin of their lips and minor inflammation but this cleared quickly. When people were taking a dose equivalent to 0.25mg/kg of their body weight, about 3.5% were affected by tiredness and minor mood changes but, overall, only 13 people stopped taking the drug because of the side effects.

And this is interesting because? Well, in the US, Accutane is considered a little controversial. This should not be surprising. The drug was originally developed as chemotherapy for the treatment of some cancers. As you can imagine, the effects on the body can be dramatic at the dosages required to stop cancer from spreading. But, when used as a treatment for acne, the lower dosages are less threatening. Indeed, if you believe the 6-year study from New Zealand, at the lowest possible dosages, you get the benefit of clear skin without almost no side effects. Well, no side effects so long as all women taking Accutane take full measures to prevent conception. Even at low dosages, this drug causes birth defects.

What’s the controversy? Well, one or two legal cases have been working their way through the courts alleging Accutane has caused major depressive illness and, in one or two instances, inflammatory bowel disease. This then defines the difference between a major developed country like the US and backward islands somewhere in the sea near Australia, or perhaps down towards the Antarctic depending on your point of view. In the US, the manufacturer is thinking of suspending sales. In New Zealand, it’s freely available to everyone with serious acne not responding to other types of treatment. Fortunately, you can buy this highly effective (but controversial in the US) drug from an online pharmacy. It really does cure almost every case of acne. There’s a tiny risk of serious side effects but, so long as you ask tell a lawyer, there’s no problem in taking it.

It’s always sad when a great story fails to stand up to the historians. We all like Longfellow’s version of the ride but, it seems, he was only one of many and never uttered the now famous warning about the Brits. Unfortunately, the modern warning will stand up to the test of history. It’s a matter of fact that there was a Boom in births between 1946 and 1964. Worse, it’s also a matter of fact that the so-called Hippie generation is more heavily into street drugs like marijuana and the legal highs of prescription drugs than any other group in the US. Ask anyone connected to the federal Substance Abuse Administration and they will tell you the same story. The number of people aged 50 and more who abuse drugs is rising fast. Why should this matter? Well, the healthcare service includes drug-treatment programs that are supposed to help people through the pain of withdrawal and to teach coping strategies for life after dependence. As the population ages, a wave of people who have consistently used a significant range of different substances is going to begin encountering health problems. You cannot abuse these substances indefinitely and avoid the adverse physical consequences. The expectation is that the treatment programs will collapse unless, at a minimum, the number of treatment facilities doubles over the next five to ten years. Worse, the older people are, the more expensive it is to treat them.

What is the main health problem going to be? As bodies age, everything slows down. In particular, the liver and kidney lose efficiency and do not clear drugs from the blood stream as quickly. If older people are also starting up new treatments, say for high blood pressure, the drugs could interact and cause major adverse side effects. There could also be problems for physicians as they try to diagnose diseases and disorders. For example, there is clear evidence that the long-term use of marijuana causes some memory loss. How is this to be distinguished from the memory problems stemming from the early onset of Alzheimer’s? At present, there is no clear evidence as to how often physicians ask their patients about substance abuse. Just as important, there is no evidence about how honestly patients answer questions about their sometimes illegal drug use. It is obvious to the federal government that there should be routine screening of all patients aged 50 or more. This would cover the spectrum of substance abuse from alcohol and nicotine to painkillers. All patients should be counselled on the need to quit. But patients may fear admissions of illegal drug use may end up on their records. At the least, this will damage their reputation, but it could result in prosecution.

In terms of prescription drugs, the most often abused are the anti-anxiety and treatments for insomnia, mainly ambien. There is a reflex in physicians when their older patients report problems in sleeping. This is an expected aspect of growing old. People lose their jobs. They start feeling socially useless. This leads to stress and some depression which interferes with normal sleep patterns. Only a few physicians are trained in geriatrics. They neglect to ask about lifestyles and just write the standard prescription for ambien. While none of this changes the facts about ambien which remains the best of the drugs to treat insomnia. It should alert all Boomers about the need to review lifestyle with their physicians and, if possible, cut down or quit nonessential drug use.
